Office Hours are 8:00am to 5:00pm, Monday thru Thursday; 8:00am to 12:00pm, Friday the office is closed. .
Please be prompt for your appointment so we can give you and our other patients the best possible care.

Referral Information:

Phenix City Pain Management does not require referrals for our patients. However, your insurance may require one. It is up to you to understand your policy requirements. We advise all potential patients to contact their insurance company to confirm coverage limitations.

Referrals from Medical Providers:

We happily accept referrals from physicians and other providers. We will work closely with you in determining a treatment plan for your patient. Because we understand that time is a critical component in the treatment of pain, we work very hard to ensure patients are scheduled for an appointment within less than 2 weeks.

Appointments:
We work by appointments to ensure your visit with us is pleasant and convenient. Please contact us at (334) 664-1969 to schedule an appointment.

If you are unable to keep your appointment or will be arriving late, we will be happy to assist you in this matter. We ask that you call our office as soon as possible so that we may reschedule your appointment.

New Patients:
Please take a moment to download our New Patient Packet.

To aid us in providing you with the best possible care, please complete the packet completely and return it to our office. Once we have begun gathering your medical records, you will be contacted concerning an appointment time.

Preparing for your appointment:
Fill out your paperwork as completely as possible. Write down any symptoms you’ve been having and for how long. Note any recent injuries. Write down key personal information, including any mental or emotional stressors in your life.

Make a list of your key medical information, including any other conditions for which you’re being treated and the names of any medications, vitamins or supplements you’re taking. When you come to your first appointment, remember to bring all medications in the original bottles.

Take a family member or friend along, if possible. Someone who accompanies you may remember something that you missed or forgot

Write down any questions to ask your doctor. Creating your list of questions in advance you can make the most of your time with your doctor.

Insurance:
Most insurance plans are accepted including private insurances, Medicare and worker’s compensation.

Payment options:
Your co-pay should be paid at the time of your visit. We accept Cash or check. There is a $35.00 non-sufficient funds fee on all returned checks.

Emergencies:
In case of emergencies after regular business hours please go to your nearest emergency room.

Patient Support:
Please allow our specially trained staff to answer your questions and concerns about your pain. Do not hesitate to call during office hours, if you have any questions regarding treatments and conditions. We are more than happy to assist you.
